Deficit Reduction in the Post-Blockbuster Era

As Washington scrambles to prepare for the largest government spending cut in modern U.S. history, no one has more at stake than the health care sector. In spite of undergoing a massive overhaul in 2010 with the passage of the Affordable Care Act (ACA), the U.S. health care system is again facing seemingly inevitable transformation as Congress moves forward with deficit reduction. For biopharmaceutical companies, this means another hard hit to the bottom line that could further exacerbate slowdowns in drug discovery and development. The biopharmaceutical industry is experiencing a bit of an identity crisis. As industry leaders tumble over the “patent cliff,” generic competitors will rapidly erode market shares long enjoyed by blockbuster products. Companies must also adjust to a changing market access environment where payers have higher expectations and more leverage to negotiate lower reimbursement rates. The still-unknown impact of ACA provisions—such as the expansion of comparative effectiveness research and establishment new panels such as the Independent Payment Advisory Board (IPAB)—have created a great deal of uncertainty about the profitability and viability of new and existing products moving forward. Despite much uncertainty, one thing is clear: the industry is undergoing a sea change where they will be expected to deliver much more with significantly fewer resources at their disposal.

Finding Shared Value in the Fight Against Non Communicable Diseases

The international community gathers for the United Nations High-Level Meeting (UN HLM) on Non Communicable Disease (NCDs) this week in New York. Many are optimistic that the UN HLM will intensify focus within the international community on NCDs and solidify the link between NCDs and economic growth. The UN HLM has the potential to be the beginning of a global drive against NCDs. But this global drive must positively engage all sectors of society for it to be successful.

Playing Tug-of-War with PDUFA

Last week, the FDA released its long-awaited recommendations for the upcoming Prescription Drug User Fee Act (PDUFA) reauthorization of 2012. PDUFA authorizes FDA to collect user fees from drug companies that file new drug applications. In return for the fees, sponsor companies are guaranteed that their applications will be reviewed in a timely manner. Under this framework, FDA funding for drug approvals is not disrupted by the annual budget and appropriations process—which has become a bit of a political circus, given current pressures to cut back on government spending and reduce the federal deficit. Industry and beltway insiders have been buzzing that the package will likely pass through Congress without serious disruption. FDA has been in negotiations with key stakeholders such as PhRMA, BIO, and other stakeholders for over a year to sort out the technical details of the package. As proposed, the recommendations do not drastically change the drug approval process the way PDUFA did following the last reauthorization under FDAAA. A few key changes in the package include:

Report: You Are What You Watch

Earlier this week, the American Academy of Pediatrics (AAP) released a policy statement on “Children, Adolescents, Obesity, and the Media.” According to the AAP, obesity represents a “clear and present danger to the health of children and adolescents.” The statement claims that “most researchers now agree that the evidence linking excessive TV-viewing and obesity is persuasive,” with dozens of longitudinal and correlational studies documenting a connection.

Improving Health Through Prevention

Leaders from the National Prevention, Health Promotion and Public Health Council hosted a press conference yesterday to release the National Prevention Strategy, the first-ever cross-sector, integrated national strategy to improve health through prevention. The document was developed with input from wide range of federal agencies and an advisory board of stakeholder groups. It has four areas of strategic focus: health and safe community environments; clinical and community preventive services; empowered people; and elimination of health disparities.

Healthy Eating: What’s Your Policy?

Late last week, at the request of Congress, the Federal Trade Commission, along with partners in an interagency working group at the USDA, FDA and CDC, released for comment a set of “Preliminary Proposed Nutrition Principles for Marketing Foods to Children Ages 2-17.” Congress tasked the working group with developing a set of principles to guide industry efforts to improve the nutritional profile of foods marketed directly to children and to tap into the power of advertising and marketing to support healthful food choices.
